ISO Medical Groups That Treat AVM in Chicago

Hi!
I am fed up with my current medical group (advocate) and found out through my insurance that I am able to switch medical groups.
I am from the Chicago area and was wondering if any of you guys have had good experience with specific medical groups? I have HMO and unfortunately cannot venture out of my medical group without paying completely out of pocket.
Any help will do!
